Do you want to be part of innovative system-wide changes currently taking place to deliver first-class acute care in a patient’s home? If so the new Hospital at Home service (H@H) could offer an exciting opportunity for you.

Hospital at Home provides the care and treatment patients expect in a hospital, in the place they call home. It brings together health professionals as a team and combines the latest health monitoring technology with the specialist knowledge and treatment of staff. Through this new way of working, patients will be able to access and receive the same level of care as if they were on a hospital ward. We call it ‘the care of hospital, the comfort of home’.

We know patients who are supported through an acute illness recover more quickly in their own homes, with less chance of de-conditioning, falls and hospital-acquired infection; and more chance of staying connected to their communities and carers. It’s better for patients and helps reduce pressure on our hospitals.

It’s one of the most exciting developments in the NHS and at MFT we are developing our service in Manchester and Trafford. This is through exciting collaboration between our community and acute services, social care and primary care. We are building on our excellent existing acute virtual ward and community Hospital at Home offers by expanding their capacity and support – starting this Winter.

The opportunity

Hospital at Home provides a fantastic opportunity for professionals who may be looking for a new challenge. You can further develop your skills and gain the exposure and experience of working in a pioneering new system. The H@H service will present valuable shared learning and development for all colleagues, together with the benefits of being part of a multi-disciplinary team, led by a patient-centred way of working.

How can you help?

Have you considered how valuable your acute or community gained skills could be in this model? Have you ever wondered about trying a role working in the community? There are opportunities available for secondment roles and consideration will be given to accommodate flexible working. Opportunities will be available within nursing, therapies, and pharmacy for both registered and unregistered staff.

The key skills we are looking for include triage, assessment, treatment & care planning for people with complex physical, medical & social needs and the ability to recognise deteriorating medical status. Experience in areas such as emergency care, frailty, respiratory, and cardiology are beneficial but not essential.
