WELCOME
Welcome to Delamere House NHS Intermediate Care unit. This welcome pack provides useful information about your inpatient stay at Delamere House. Staff will be happy to answer any questions you may have about the pack and clarify any issues with you.
WHAT IS INTERMEDIATE CARE?
Intermediate Care is a service aimed at meeting the needs of people who require a period of inpatient rehabilitation to improve mobility and general level of functioning along with a coordinated discharge plan to enable safe discharge into the community. This service provides short-term rehabilitation to enable patients to regain their optimal levels of independence.
WHY ARE YOU HERE?
You have consented to be admitted to Delamere House for a period of rehabilitation to help you improve your independence with everyday tasks such as washing, dressing, and walking.
ABOUT THE UNIT
Delamere House is an NHS Intermediate Care facility with 23 beds, situated within Gorton Parks Care Home. Your general care and support on the unit will be provided by NHS/LCO staff and carers. Your nursing, therapy and discharge planning will be provided by NHS/LCO staff.
WHAT CAN WE OFFER YOU
We will work with you to set realistic personal goals to achieve before you leave Intermediate Care These goals may relate to your personal care activities such as washing and dressing or other domestic activities of daily living such as cooking for yourself. They may include goals related to your mobility, for example
being able to manage the stairs; and to your health conditions, for example being able to manage your own medications.
The length of time that you will stay at Intermediate Care will be individual to your needs, therapy goals and discharge plan. You will have a named Keyworker team who will coordinate your care, support, therapy and discharge arrangements. The team will work towards achievable goals to facilitate a safe discharge and arrange for follow up input in the community if further rehab goals have been identified.
WHO ARE THE STAFF MEMBERS WORKING ON THE UNIT?
We are a multi-disciplinary team comprising of:
Nurses, Advanced Clinical Practitioners, Occupational Therapist, Physiotherapist, Social Worker Rehabilitation Assistants, Clinical Support Workers, GPs – Monday to Friday and Pharmacist. The multi-disciplinary team meets weekly and will review health, care, therapy goals and discharge planning for our patients.
We work closely with other community services such as a Speech and Language Therapy, Podiatry and dieticians.
WHAT HAPPENS WHEN I ARRIVE?
When you arrive, you will be shown to your bedroom, and you will be welcomed by members of staff who will introduce themselves and the unit and do some initial assessments with you. You will be informed of the name of your Key Worker and given this welcome pack. Staff will make a note in your file of your belongings. We ask that you do not bring in valuables or money.
THERAPY INPUT
We aim to offer Physio and Occupational Therapy 2-3 times weekly. The Therapy staff will set up a treatment plan with yourself which all nursing, rehab staff and CSW’s will follow so that your rehabilitation would be happening 24/7. You will be offered 1:1 assessment and treatment sessions with physiotherapist/OT. It is important that you engage in your rehabilitation to optimise your opportunity to gain independence. There are also therapy groups regularly organised for patients to participate in.
Therapy and care staff encourage patients to participate fully in personal care tasks and do as much for themselves as possible as this will promote independence in preparation for discharge home.
HOW LONG AM I GOING TO STAY HERE?
Your stay will be identified based on your needs, rehab goals and discharge planning and we will aim to get you home as soon as it is safe and appropriate to do so. We will always be planning your discharge from the moment you arrive and will never advise that you remain in intermediate care longer than is necessary.
DISCHARGE PLANNING—HOW WILL MY DISCHARGE BE PLANNED?
You will have an allocated Keyworker who will coordinate your discharge and then work alongside you to plan and arrange a safe and supported discharge into the community. The Keyworker will work with you to set goals, a therapy plan and discharge arrangements with involvement from yourself, your family and carers. We will complete access, home and/or discharge visit with you to ensure your environment is suitable for your return home and we will identify any possible adjustment/equipment provisions. We will work with a range of other services such as Social Services, District Nurses, Reablement etc. to arrange an individual discharge plan and any necessary follow-up support.
VISITORS
Your relatives, friends and carers are welcome to visit you. There are no fixed visiting hours, but we ask that visitor’s avoid meal times. Relatives can contact the unit on 0161 230 3010 to see how you are or talk to a member of staff.
MEALTIMES ARE:
Breakfast 8.30am to 10.30am
Lunch 12.30 to 13.30pm
Evening meal 4.30pm to 5.30pm
You will be expected to go to the dining room for your meals, this is all part of your therapy and rehabilitation journey. You will be provided with three meals each day and will be offered a choice of menu. Hot and cold drinks are also available throughout the day. You may also be offered the opportunity to prepare some meals for yourself as part of your rehab care plan.
DIETARY REQUIREMENTS
Please inform a member of staff if you have any special dietary requirements that you have from a religious, health or personal preference perspective. Intermediate Care operate a ‘Protected Mealtime’ Policy and would appreciate it if visitors do not attend during designated mealtimes.
TELEPHONES
There is a patient phone that can be made available so patients can contact family.
WHAT IS IN MY ROOM?
Bed, wardrobe, a sink, cabinet, armchair, commode (for night-time use only), over bed table, a call bell that you can use if you require assistance. There are also accessible televisions is all rooms. Your room will be cleaned daily. We ask that you keep your room tidy to reduce risk of falls.
TOILET FACILITIES
Male and female toilets are situated at the end of each corridor. Each room has a commode for night-time use if needed.
SMOKING
Smoking is not allowed in the building. There is a designated outside smoking area if required. If you would like advice on giving up smoking, please ask a member of staff who will be happy to help.
LAUNDRY
We ask that patients bring in a supply of both day and night wear, underwear, toiletries, any continence products used at home and suitable footwear. This is to ensure that patients can engage fully in therapy and in order that we can support you to return to managing as you did before. As we are a community resource, we do not have the facility to launder patients’ individual clothes and ask that family / friends attend to laundry during your stay.
CAN I LEAVE WHEN I WANT TO?
You are free to leave the unit as and when you choose. Please let staff know if you are going to leave the unit to visit friends or have any appointments to attend. We are unable to provide escorts for patients attending appointments and would encourage family / friends to support if possible. We are, however, able to arrange transport to enable you to attend any health-related appointments that may arise during your inpatient stay. We do require 24-48hrs notice to arrange transport for hospital appointments. We are also very happy to encourage families to support their loved ones with transportation to attend appointments and for discharge home.
INFECTION CONTROL
We work hard to ensure we provide a safe and clean environment for everyone. All staff have regular training on infection prevention and control. We promote and monitor the use of alcohol gel and hand washing. Managers work closely with the infection prevention and control team and the ward cleaning staff to maintain high standards of cleanliness. You will be expected to maintain good personal hygiene during your stay. We also ask that visitors wash their hands on entry to and exit from the building, using the handwashing facilities situated by reception.
RELIGIOUS AND CULTURAL NEEDS
We endeavour to meet the religious and cultural needs of all our patients and will work with patients and their families to achieve this.
IF YOU NEED TO RAISE ANY CONCERNS / QUERIES
Should you have any concerns about your admission to intermediate care, please feel free to discuss these with staff on the unit or our senior team. We will do our best to alleviate any concerns as soon as they are brought to our attention. If, for any reason we are unable to do so then we can provide you with details to access the trust PALS service for further support.
