Short Term Intervention Service
The service is a Reablement Service for adults with Learning Disablility. It’s a short term service which may be provided for up to 12 weeks and provides support provision city wide. The service aims to support people gaining confidence and skills to manage practical daily tasks, which will enable them to live as independently as possible. As well as promoting independence, the service also provides a period of assessment to identify any other services you may need in the longer term.
The Short Term Intervention Service provides support to vulnerable adults diagnosed with a Learning Disability. We support to break down daily / household and social tasks to assess independence and identify needs, looking at assistive technology to enable the person to complete as much as possible for themselves to meet their goals.
The team work to support vulnerable adults and their families to access other health and social care services with the aim of achieving positive outcomes. The team provides person centred support through a strength-based approach, setting achievable goals, building confidence, moral and sustainability with the use of technology.
Referrals are received from the citywide Learning Disability Teams and from Hospitals. Citizens have to be a Manchester resident.
The service has a CQC registered manager. The team consist of team leaders who support with:
- Managing staff
- Referrals
- Assessment and reviews
Our Short Term Intervention support workers visit customers up to 4 times a day, between the hours of 8am-10pm, 7 days a week. Whilst providing practical support, they also provide emotional support and complete monthly progress reports for all our customers.
The service has access to an occupational therapist who works alongside our support workers to tailor individuals’ goals and assess and provide equipment to support independence.
It is essential that the initial assessment is completed with the customer in their own home by the allocated worker. Customers and their families are involved in all assessments and reviews.
The monthly progress reports are completed with our support workers. Customers are reviewed at week 6 to review goals and to identify and establish a plan going forward.
Short Term Intervention operates Monday – Sunday, 8am-10pm.
