District Nursing Service – Days – Citywide
The District Nursing service delivers holistic quality assured nursing care to those aged 16 and over living in Manchester to ensure that patients and their carers are able to maximise their independence and to remain at home whenever possible.
We provide nursing care to patients in their own home who are unable to access a treatment room, or require nursing care that is most appropriately delivered in their own home.
We provide a wide range of nursing care packages that offer a health-promoting and educating, curative, restorative, supporting and or palliative function personalised to meet the health needs of each individual and their carers and family.

- Palliative care and management of symptom control
- Wound Care
- Skin care and pressure area management
- Diabetes care and management of associated complications
- COPD (Chronic Obstructive Pulmonary Disease)
- Neurological and degenerative conditions
- Heart failure
- Urological conditions
- Post-operative wound care
- Acute (post-operative) and chronic eye conditions
- Gastrointestinal problems
- Circulatory conditions
The District Nursing Service places the patient at the centre of the care planning and decision making process using a single assessment process. The District Nursing Service works in close partnership with GP practices, secondary care, community services, the Local Authority and others to meet the nursing needs of adults in Manchester. Also providing educational placements for other health care professionals, including both pre and post registration nurses.
Care packages include:
- Palliative care and symptom control
- Rapid response assessments
- Nursing assessments, for CHC, funded nursing care or residential care
- Medication administration eg. oral, eye, via gastrostomy tubes, injection, central lines.
- Acute (post-operative) and chronic eye conditions
- Diabetes care
- Urological conditions, including catheter care, continence promotion.
- Gastrointestinal problems e.g. support with stoma care, gastrostomy feeding regimes and medications
The District Nursing Service operates an open referral system and is the main provider of home based community nursing care to adults in Manchester to prevent hospital admissions, to support early discharge from hospital and to receive nursing care in their own home including residential homes or a clinic setting. We also support end of life care in people’s homes.

Opening hours
The District Nursing Service is available 365 days per year, from 8am until 5:30pm, and is supported by the District Nursing Evening and Night Service in Ward 30 in MRI from 5pm until 8am.
