District Nursing Service – Evenings and Nights – Central Manchester

The Evening and Night District Nursing Service delivers holistic and quality assured nursing care to those aged 16 and above living in Manchester, to ensure that patients and their carers are able to maximise their independence and remain at home whenever possible.

Our Aim

We work to prevent hospital admissions, to support early discharge from hospital, to allow patients to die in their own home when this is their preferred place to die, and to receive nursing care in their own home, including residential homes when this is required.

Who we help

Aged 16 and above living in Manchester

How we help

Covering all of Central Manchester, the Evening Service and is led and managed by an experienced District Nurse Assessor and consists of three teams, between the hours of 17:30hrs until 08:00hrs. The Overnight Service is managed by one team including an experienced District Nurse Assessor and Health Care Support Worker from 22:30hrs until 08:00hrs for 365 days of the year.

We provide a range of nursing care packages that offer a health promoting and educating, curative, restorative, supporting and or palliative function personalised to meet the health needs of each individual and their carers and family.

We place the patient at the centre of the care planning and decision making process using a single assessment process and the patient held Manchester Adult Record to support this process.

We work in close partnership with GP practices and the Out of Hours GP Service, secondary care, community services, the Local Authority and others to meet the nursing needs of adults in Manchester. We also provide educational placements for other health care professionals, including both pre and post registration nurses.

We liaise closely with the Day District Nursing Teams to provide a seamless District Nursing Service.

Referral Information

We operate an open referral system providing ‘out of hours’ home community nursing care to those aged 16 and above and resident in Manchester and those registered with previous fund holding GP practices living outside the Manchester boundary.
