Palliative care is patient centered rather than disease focused, death accepting but also life enhancing, a partnership between the patient and family, concerned with managing rather than curing.

Referrals are accepted by email, telephone (Monday – Friday), face to face and post. Please telephone the team when making urgent referrals.

Referrals are accepted from:

• GPs
• Colleagues, fellow professionals, voluntary, statutory and non-statutory organisations
• Individuals and families (self-referral)
• Hospital Paediatricians
• Community Paediatricians
