The Community Nursing Service delivers care to people aged 16 and over who are housebound and registered with GPs in South Manchester.
Our nurses provide palliative care for patients with progressive illnesses and a limited prognosis when home is the patient and carer’s preferred place to die.
The service also supports urgent response to crisis situations, with the aim of preventing hospital admissions and supporting early discharges from hospital. Where patients need planned appointments for medical care, the Community Nursing Service co-ordinates a scheduled appointment system.
