Adult Social Care City Wide Carers Team

Adult Social Care Commissioners in the Manchester Local care Organization (MLCO) worked in partnership with the established Citywide Carers Manchester Network, made up of 18 VCSE dedicated Carer organizations and statutory services like Manchester City Council, NHS and Greater Manchester Mental Health Foundation (GMMH); Creating the umbrella brand ‘Carers Manchester’. There are 5 main workstreams within the pathway  

  • Carers Manchester Contact Point (CMCP), Carer Network Co-ordination, 
  • Communications & Marketing, VCSE Locality Partnerships and Learning & development for Carers.  
  • Carers Manchester Contact Point is an externally commissioned helpdesk service and is the first point of contact for Carers in Manchester. 
  • CMCP Advisors can provide Information, Advice & Guidance to Carers and link to services in the community  
  • CMCP Advisors will refer on to statutory services for a Carer’s Assessment, as well as provide access to emergency funding and benefits advice, if required.

MCC Carers Team complete Strength-Based Assessment which is an opportunity that can help Carers to understand their situation and the needs that they have, their strengths and capabilities, what is available in the community to support them, how to reduce or delay needs from escalating and how to access timely support if they require it.

The Carers Team is a City-Wide service working closely with partners in health, INT’s and the neighbourhoods. 

The Carers Team consists of dedicated team of Social Care Assessors (SCA) who complete Carers Assessments across the city for carers in Manchester to access support, guidance and signpost information to services identified as part of the carers assessment. 

The Carers Team provide a Statutory Carers Assessment for Carers in Manchester. 

The Carers Team will also identify any support required for the cared for person and send referrals to the appropriate teams and services. 

Criteria for the Carers Team 

  • The cared for person needs to be Manchester resident 
  • Over 18 years old and look after another person 
  • Carers don’t have to live with the person they care for  
  • Carers can work full time or part-time or claim benefits 
  • Carers sharing caring responsibilities with another person.

Carers who don’t meet the criteria 

If the carer is under 18, we link in with Early Help and Children’s services to seek support, advice and guidance for a young carer.

If the cared for has Learning Disabilities, we liaise with a dedicated LD Social Care Assessor. 

If the cared for has diagnosed Mental Health and under 65 we liaise with dedicated GMMH Carers Assessors.

What is a carers assessment?  

A carers assessment looks at the physical and emotional wellbeing as a carer who provides support to a cared for person.  

The Carers Social Care Assessment Officer (SCA) will complete a carers assessment via an in-depth conversation to learn what support or services are available to support the carer which best meets their identified needs, wants and wishes.  

This includes providing advice, guidance, recommendations and signposting to other universal or specialist services that could better support a Carer and Cared for. 

What happens during the assessment. 

The SCA will complete a strengths-based assessment, which identifies the impact the role as a carer has on the carer and what current support is already in place.  

The SCA can also refer the carer or the cared for, for equipment, assistive technology or to Manchester City Councils Integrated Neighbourhood Teams.

The SCA will inform the carer based on the information provided if they have met the criteria for a One-off carer’s payment, then complete a support plan to agree what the budget will be used for to support the carers wellbeing.

A family member, a friend or an advocate can be present if the carer prefers. 

Interpreters will be arranged as required.  

What happens after the assessment.  

The SCA will complete the relevant assessment paperwork on Manchester City Councils electronic system and will ensure consent has been agreed. 

If the carer has met the criteria for a one-off carer’s payment, information will be sent to the finance team to process the payment.  

Copies of the assessment will be emailed or posted out if requested.  

A survey will be email/posted to the carer, which give them the opportunity to feedback if they were satisfied with our service. 

The survey is a tool we use to continually appraise or service and is designed to help us to improve our services.   

The SCA will ordinarily review and follow up their assessment within 4-6 weeks to enquire if any Advice/ Guidance or signposting has proved beneficial and will enquire if the one-off-payment where applicable has been received.   

We will also collect data on what the one-off carers payment were used for and record this on the file and use this with Commissioners to horizon scan for opportunities to develop services further.  

If payment has not been received the SCA will pursue this on behalf of the Carer.

The Carers team consists of 7 Social Care Assessors and a Team Manager.
The Carers Team has a duty function which is monitored daily. 
All referrals to the team come via the Contact Centre work on our electronic system to the Carers Incoming work tray. 

On receipt of a referral for a carers assessment MCC will endeavour to allocate the case for assessment at the earliest opportunity.
The Carers Team respond to all request for assessment with a contact being made within 48 hours and advise the carer of waiting times for assessment in order to manage the expectations of the carer. 

The Carers Team operates between the hours of 9am – 4.30pm, Monday to Friday.
