The Positive Futures team is a new team that sits within the Reablement service, the team supports individuals from a wide cohort, this includes people with a learning disability, autism, neurodiversity, mental health, substance misuse, early-stage dementia, and residential care. It is a short-term service that aims to support people within 12 weeks in their own home and community.  The service aims to support people gaining confidence and skills to manage practical daily tasks and engagement, which will enable them to live as independently as possible. As well as promoting independence, the service also provides a period of assessment to identify any other services that may be needed in the longer term.

The Positive Futures team provides support to individuals across the city of Manchester who have a diagnosis of a learning disability, autism, neurodiversity, mental health, early-stage dementia or who needs support with substance misuse. An individual may have multiple diagnosis and the team endeavours to support these individuals to achieve goals with a support worker and also supported through technology enabled care (TEC) to promote independence and reduce care needs.

The team work to support vulnerable adults and their families to access other health and social care services with the aim of achieving positive outcomes. The team provides person centred support through a strength-based approach, setting achievable goals, building confidence, moral and sustainability with the use of technology.

Referrals are received from different teams within Manchester City Council via their social worker or social care assessor. We also receive referrals through hospitals and the Greater Manchester Mental Health team. Citizens must be a Manchester resident.

The service has a CQC registered manager. The team consist of team leaders who support with:

  • Managing staff
  • Referrals
  • Assessment and reviews

Our Positive Futures Team support workers visit customers up to 4 times a day, between the hours of 8am-10pm, 7 days a week, 365 days a year. Whilst providing practical support, they also provide emotional support and complete monthly progress reports for all our customers.

The service has access to Reablement occupational therapists who works alongside our support workers to tailor individuals’ goals and assess, provide support around techniques and provide equipment to support independence.

It is essential that the initial assessment is completed with the customer in their own home by the allocated worker to ensure a person-centred approach. Customers are involved in all assessments and reviews; customers can have family present if they consent to this.

Monthly progress reports are completed with support workers and the customers to discuss what has been going well, review of goals and establish a future plan. Any updates the support worker will then inform the allocated worker so the support plan can be updated to reflect this.

Positive Futures operates Monday – Sunday, 8am-10pm, 365 days a year
