Manchester’s Reablement Service is a citywide service, based at 3 locations in the North, Central and South areas. Reablement are a community based service who provide a period of support and assessment for up to 6 weeks in an individual’s own home, to maximise independence, choice, safety and quality of life. As a preventative service, we aim to reduce reliance on care services, and prevent delays in discharge from hospital.

The service aims to work with adults to achieve their goals and reduce reliance on long term care services, preventing admission or re-admission to hospital. Referrals are received from our Discharge to Assess (D2A) and community teams for Manchester residents.

The team provides person centred support through a strength-based approach, setting achievable goals around personal care, meals and medication through building confidence and the use of technology. Recent end of service outcomes for reablement indicated that 70% of our customers ended as independent with a further 9% reducing their care support.

Each locality has a CQC registered manager. The teams consist of team leaders who support with

  • Managing staff
  • Referrals and out of hours support
  • Assessment and reviews

Assistant Team Leaders support with staff management, medication and audits of the service.

Our reablement support workers visit customers up to 4 times a day, between the hours of 8am-10pm 7 days a week. Whilst providing practical support they also provide emotional support and complete weekly progress reports for all our customers.

Each locality has an occupational therapist who works alongside our support workers to tailor individuals’ goals and assess and provide equipment to support independence.

The initial assessments are completed in the customers home by our D2A assessors or an individual social worker/social care assessor. Customers are involved in all assessments and reviews and weekly progress reports are completed with our support workers. Customers are reviewed at week 3 to review goals and establish long term support needs. The service is provided for up to 6 weeks.

Reablement operates Monday to Sunday between the hours of 8am-10pm.
